The Case Manager/(QIDP) is responsible for assessing, monitoring, documenting and ensuring the provision of quality care to people with intellectual and developmental disabilities.
This person will plan, coordinate, integrate, and monitor the active treatment process that moves individuals toward independence utilizing a person-centered approach to assist the individuals in fulfilling their desires and needs for an independent life.
The role requires maintaining continued awareness of new developments in programming, active treatment, and other related areas for persons with developmental disabilities.
Other major functions include participating in committees, performing investigations, writing reports, and reviewing trends and patterns.
Coordinating with nursing staff and medical personnel for health care needs for all individuals will also be key.
QUALIFICATIONS: Must have a bachelor's degree in social work or school of social work, sociology, special education, rehabilitations counseling, or psychology.
Registered nurses and physicians also qualify to serve as a QIDP.
Must have at least one-year experience working directly with persons with intellectual disability or other developmental disabilities.
Technology skills and computer literacy required.
Must have strong communication, writing, time management, and organizational skills.
Need to be proficient in Microsoft Office suite, particularly Word, Excel, and Outlook.
Must be self-motivated and have the ability to work with minimal supervision and be able to handle highly confidential information.
Must possess strong leadership abilities and a willingness to work effectively with other administrative staff, ancillary services personnel, and habilitation program staff.
